Registration Payment
Full payment is due at the time of registration to secure a participant’s spot. Please contact Lisa to determine if there’s a spot available before payment is made. Lisa can be reached at nagy_123@sympatico.ca or 365-292-5227.
The registration payment can be made by Interac e-transfer. The email address will be provided to you once Lisa determines if there’s a spot available.
There's a form for parents/guardians to sign at the time of registration acknowledging the Release of Liability, Medical Emergency, and Session Cancellation Due to Shortage of Staff or Severe Weather terms. The online waiver/registration form is located under the Registration tab.
[Cheque or cash on the first day of the session are also payment options (although Interac e-transfer is preferred). Please contact Lisa to ensure there’s a spot for the participant.]
Release of Liability
As the legal parent or guardian, you agree to release, waive, discharge, and hold harmless Burlington Soccer Circle, its owners and operators, from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, that may be sustained by the participant, while in or upon the premises or any premises under the control and supervision of Burlington Soccer Circle, its owners and operators or enroute to or from any said premises.
MEdical emergency
As the legal parent or guardian, you give permission to Burlington Soccer Circle, its owners and operators, to take all responsible steps to respond to a medical emergency and to seek medical treatment in the case of injury in the event they are unable to reach a parent or guardian. You hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
session cancellation due to shortage of staff or severe weather
We will try our best to run all sessions as scheduled; however, there may be a time when we will need to cancel a session due to staff shortage or severe weather. Please check your email regularly, especially on days that your child/participant has a session. A make up session credit will automatically be applied to your child’s file for future use.
